Dementia Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Serves as the main contact for caregivers, families, and seniors dealing with dementia.

• Actively assists caregivers, families, and seniors with dementia to improve quality of life and ensure home safety.

• Provides education and support regarding dementia, its symptoms, caregiving strategies, and expectations using our resource playbooks and center.

• Evaluates unmet care requirements, including clinical or medication challenges, behavioral concerns, safety threats, and the psychosocial well-being of both the caregiver and the patient.

• Assesses needs, sets goals, and crafts personalized care plans with the help of the clinical team.

• Investigates and connects clients with local community resources.

• Manages medication reconciliation and assists with medication management approaches.

• Aids in the coordination of medical care by facilitating referrals and compiling updated medical records.

• Collaborates and triages complex issues with our multidisciplinary team, which includes licensed clinical professionals (e.g., nurse, social worker, physician, pharmacist).

• Contributes to the creation and maintenance of content for informational resources, ensuring accuracy and relevance.

• Provides documentation and supports general administrative duties, including record management, correspondence handling, and adapting to new tasks as they emerge.

• Exhibits flexibility and a readiness to engage in various tasks as needed in a dynamic startup environment, contributing to the team effort across different operational areas.

⛳️ Requirements

• Bachelor's degree, preferably in social work or psychology.

• Proficient with technology and capable of performing this role virtually.

• Fluent in English with strong communication skills.

• Able to collaborate effectively in a multidisciplinary team setting.

• Experience in building rapport and fostering collaborative relationships with patients and their families.

• Capable of managing difficult patient interactions with empathy and professionalism, even in challenging or emotionally charged situations.

• Bonus qualifications: Master of Social Work (MSW).

• Experience in healthcare, particularly with dementia.

• Multilingual abilities, especially in Spanish.
