This is a fully remote position, open to applicants in United States.
β’ Serves as the main contact for caregivers, families, and seniors dealing with dementia.
β’ Actively assists caregivers, families, and seniors with dementia to improve quality of life and ensure home safety.
β’ Provides education and support regarding dementia, its symptoms, caregiving strategies, and expectations using our resource playbooks and center.
β’ Evaluates unmet care requirements, including clinical or medication challenges, behavioral concerns, safety threats, and the psychosocial well-being of both the caregiver and the patient.
β’ Assesses needs, sets goals, and crafts personalized care plans with the help of the clinical team.
β’ Investigates and connects clients with local community resources.
β’ Manages medication reconciliation and assists with medication management approaches.
β’ Aids in the coordination of medical care by facilitating referrals and compiling updated medical records.
β’ Collaborates and triages complex issues with our multidisciplinary team, which includes licensed clinical professionals (e.g., nurse, social worker, physician, pharmacist).
β’ Contributes to the creation and maintenance of content for informational resources, ensuring accuracy and relevance.
β’ Provides documentation and supports general administrative duties, including record management, correspondence handling, and adapting to new tasks as they emerge.
β’ Exhibits flexibility and a readiness to engage in various tasks as needed in a dynamic startup environment, contributing to the team effort across different operational areas.
β’ Bachelor's degree, preferably in social work or psychology.
β’ Proficient with technology and capable of performing this role virtually.
β’ Fluent in English with strong communication skills.
β’ Able to collaborate effectively in a multidisciplinary team setting.
β’ Experience in building rapport and fostering collaborative relationships with patients and their families.
β’ Capable of managing difficult patient interactions with empathy and professionalism, even in challenging or emotionally charged situations.
β’ Bonus qualifications: Master of Social Work (MSW).
β’ Experience in healthcare, particularly with dementia.
β’ Multilingual abilities, especially in Spanish.
β’ Contribution towards Health Insurance.
