• Offering support and encouragement to patients and families who are currently experiencing or have recently faced a behavioral health crisis.
• Manage case referrals, evaluate resource needs and care gaps, review case progress, and decide on case closure.
• Aid patients and families in achieving their individual recovery objectives through psychoeducation and connections to community resources.
• Evaluate social determinants of health and obstacles to care, which may include housing, income, food insecurity, and transportation requirements.
• Link patients and families with support groups, community networks, and resources aimed at promoting mental wellness.
• Collaborate with interdisciplinary team members to facilitate the treatment plan goals for the patient and family.
• Coordinate care management with agencies involved with the child, including DCFS, educational systems, external care team members, and more.
• Engage in ongoing scheduled consultations with an interdisciplinary team to track patient progress.
• Execute additional duties as assigned.
• A Master’s degree in Social Work (MSW) is required.
• Bilingual in English and Spanish is preferred.
• A minimum of 2 years of case management or relevant experience is necessary.
• Experience with team-based care and strong collaboration skills are essential.
• Background in child and adolescent case management, school-based care, inpatient settings, and/or community mental health is required.
• Familiarity with acute behavioral health needs, including suicidality, homicidality, and severe persistent mental illness is crucial.
• Strong working knowledge of community resources is expected.
