Continuous Care Coach

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Oversee the patient caseload and maintain communication as often as necessary.

• Engage and communicate with patients, their family members, guardians or caretakers, surrogate decision-makers, and/or other professionals regarding care aspects.

• Coordinate communication with home health agencies and other community services that the patient utilizes.

• Gather health outcomes data and complete registry documentation.

• Educate patients and/or family/caregivers to promote self-management, independent living, and daily activities.

• Assess and provide support for adherence to treatment regimens and medication management.

• Identify available community and health resources.

• Facilitate access to necessary care and services for the patient and/or family.

• Continuously review patient status, including examination of laboratory and other studies not included in an E/M service.

• Develop, communicate, and maintain a comprehensive or disease-specific care plan.

• Utilize motivational interviewing techniques to evaluate and address any barriers that patients may face, working holistically to help resolve these issues.

• Must be willing to work flexibly; however, availability during normal business hours in patients’ time zones is required.

• Capable of interacting with up to 15 patients daily to provide encouragement, empathy, and support.

• Maintain regular communication with other Team Leads, Managers, Directors, and relevant stakeholders to foster healthy information exchanges.

• Perform additional duties as assigned to further Lumata Health’s goals and objectives.

⛳️ Requirements

• Minimum of 2 years of work experience in eye care is preferred.

• MUST possess an active Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) designation - Required.

• Proficient in using Electronic Medical Record (EMR) systems is preferred.
