Consultative Pharmacist

This is a fully remote position, open to applicants in Arizona, +3 more states.

📋 Description

• The Consultative Pharmacist 2 engages with patients, pharmacies, and healthcare providers to ensure optimal medication therapy for patient care.

• Provides guidance on the safe, precise, and timely use of medications.

• Develops, implements, assesses, and modifies medication care plans as necessary.

• Oversees medication therapy and makes modifications when required.

• Serves as the medication specialist in targeted proactive health initiatives aimed at enhancing health outcomes for patients with specific conditions.

⛳️ Requirements

• Bachelor's Degree.

• A valid pharmacist license in good standing in OH, AZ, FL, or TX.

• Communicates with CenterWell Pharmacy patients via telephone.

• Offers counseling and guidance.

• Investigates patient inquiries and concerns.

• Promotes positive service and pharmaceutical results.
