📋 Description

• Deliver a member-focused, evidence-informed care model across various products.

• Assist members with significant needs in navigating the complexities of healthcare.

• Determine each member’s care objectives and coordinate necessary services.

• Link members to community resources.

⛳️ Requirements

• An Associate's or Bachelor's degree in Nursing.

• Over 5 years of experience in clinical or acute care settings.

• A current, unrestricted RN license in the state of residence.

• Certification as a Case Manager (CCM) is preferred, or a commitment to obtain it within two years of hiring.
