This is a fully remote position, open to applicants in New Hampshire.
• Provide administrative assistance to the Foster Care/DCYF team.
• Aid Care Managers and Care Navigators in emailing and updating monitoring care plans.
• Maintain the assigned monitoring care plans.
• Conduct member outreach to facilitate comprehensive assessments.
• Connect members with community resources that support their care management journey.
• Inform members about available community resources and health education as necessary.
• Assist members in accessing community and care resources.
• Organize community outreach resources and enhance awareness of care/services.
• Address member inquiries regarding community and care resources.
• Facilitate member correspondence and educational materials.
• Elevate community resource requests and inquiries to management.
• Document and maintain community resources in compliance with health plan requirements.
• Support nurses and social workers with outreach, community education, evidence-based guidance, and member support.
• Execute non-clinical general health assessments and refer members to suitable services.
• Carry out non-medical assessments and address concerns on behalf of members.
• Perform telephonic and/or in-person outreach to reach individuals and families who are hard to contact.
• Make visits to individual homes and/or community organizations as required.
• Undertake additional duties as assigned.
• High School diploma or GED is required.
• 2 - 4 years of relevant experience is preferred.
• Familiarity with Social Determinants of Health (SDOH) barriers is essential.
• Capability to conduct non-clinical general health assessments.
• Proficiency in conducting non-medical assessments, including home safety, community/environment resources, transportation, and employment.
• Ability to perform telephonic and/or in-person outreach.
• Adherence to all policies and standards is mandatory.
