Community Outreach Sales Rep – Patient Engagement Specialist

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Execute extensive outreach to potential patients, fostering relationships and engaging in meaningful conversations through a consultative approach.

• Effectively articulate the advantages and value of ConcertoCare's offerings, addressing inquiries and overcoming enrollment challenges.

• Propel patient enrollment by assessing needs, addressing objections, and encouraging patients to take action.

• Implement innovative outreach techniques, such as phone calls, in-person meetings, and community engagement, to reach hard-to-contact patients.

• Oversee a pipeline of potential patients and prioritize tasks to optimize enrollment chances.

• Document outreach efforts, enrollment status, and patient interactions in Salesforce and electronic medical record systems.

• Arrange initial patient appointments and coordinate necessary intake procedures to facilitate a seamless onboarding process.

• Address incoming inquiries from potential patients and their caregivers, providing information on available services and guiding them through the enrollment process.

• Work in tandem with clinical and operational teams to eliminate barriers and ensure effective patient engagement.

• Uphold the highest levels of professionalism, patient confidentiality, and compliance with regulations.

⛳️ Requirements

• At least 1 year of experience in a healthcare or patient-focused role.

• A minimum of 1 year of experience in customer service.

• Preferably possess sales experience with a proven track record of meeting sales targets.

• Experience in appointment scheduling.

• Background as a CNA or MA is advantageous.

• Experience in a call center environment is preferred.
