This is a fully remote position, open to applicants in Kentucky.
• Foster the organization's social community through advocacy initiatives and active engagement with Kentucky enrollees as well as the broader community.
• Enhance the visibility and reputation of Humana Healthy Horizons' products and services within the community.
• Engage with community members through various interactions.
• Develop strategies to expand communities and strengthen connections among community members.
• Attract new participants to the community.
• Conduct in-person assessments and/or create care plans with consumers.
• Execute core responsibilities at one or more locations outside of Humana.
• Travel 60–80% within a 50-mile radius of Northern Kentucky.
• Work Monday to Friday from 8 AM to 6 PM Eastern Time, with flexibility depending on business needs.
• Safeguard member PHI/HIPAA information while working in a designated area.
• Participate in occasional training sessions or meetings at Humana's office as required.
• Must reside in the Northern Kentucky area and within a 50-mile radius of Northern KY.
• Two or more years of relevant human services experience OR recent experience in a similar role, working directly with consumers to conduct assessments and/or develop care plans.
• At least one year of experience in community outreach and/or marketing and sales.
• Community Health Worker Certification must be achieved within six months of the hire date.
• Strong written and verbal communication skills.
• Proficient in Microsoft Office applications, including Excel, Word, and PowerPoint.
• Familiarity with various electronic information applications/software programs.
• Must complete a tuberculosis screening if selected.
• Valid state driver's license is required.
• Proof of personal vehicle liability insurance with minimum limits of 100,000/300,000/100,000.
• Home internet connection with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
• A dedicated workspace free from interruptions to ensure the protection of member PHI/HIPAA information.
• Must be available Monday to Friday between 8 AM and 6 PM Eastern Time and be adaptable based on business needs.
• Preferred: Bachelor's Degree in Social Work or a related health field.
• Preferred: Ability to speak Spanish fluently.
• Preferred: Experience in Home Health.
• Preferred: Knowledge of community health and social service agencies, along with additional community resources.
