This is a fully remote position, open to applicants in Washington.
β’ Deliver thorough medication management, encompassing reconciliation, dose optimization, adherence support, and drug-level evaluations in accordance with evidence-based practices.
β’ Collaborate with physicians, specialists, and multidisciplinary teams to develop tailored care plans under a Collaborative Practice Agreement (CPA) and enhance medication coordination.
β’ Assess therapy effectiveness and safety, identify opportunities for optimization, and intervene as necessary.
β’ Contribute to quality improvement and Risk Evaluation and Mitigation Strategies (REMS) initiatives.
β’ Offer guidance to operations by aiding colleagues with complex prior authorizations, enhancing workflows, educating patients, and fostering continuous clinical development.
β’ Possess a Doctor of Pharmacy (PharmD) degree.
β’ Must be a licensed pharmacist in the state of Washington.
β’ Candidates must satisfy one of the following criteria:
β’ Completion of a PGY2 Ambulatory Care Residency.
β’ Completion of a PGY1 Residency and a minimum of 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA).
β’ At least 3 years of ambulatory practice with experience in disease state management.
β’ Strong communication, organizational, and technological skills.
β’ Demonstrated empathy, integrity, and a team-oriented approach.
