• Deliver comprehensive medication management, encompassing reconciliation, dose optimization, adherence support, and drug-level evaluations in accordance with evidence-based practices.
• Collaborate with physicians, specialists, and interdisciplinary teams to establish personalized care plans under a Collaborative Practice Agreement (CPA) and facilitate effective medication coordination.
• Assess therapy effectiveness and safety, identify areas for optimization, and intervene as necessary.
• Contribute to quality improvement and Risk Evaluation and Mitigation Strategies (REMS) initiatives.
• Offer recommendations to operations by assisting colleagues with complex prior authorizations, enhancing workflows, educating patients, and supporting ongoing clinical development.
• Possess a Doctor of Pharmacy (PharmD)
• Must hold a pharmacist license in California
• Candidates must fulfill one of the following criteria:
• Completion of a PGY2 Ambulatory Care Residency
• Completion of a PGY1 Residency along with a minimum of 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
• At least 3 years of ambulatory practice with experience in disease state management
• Excellent communication, organizational, and technical skills
• Demonstrated empathy, integrity, and a team-oriented approach
