This is a fully remote position, open to applicants in Virginia.
• Engages in all facets of the medication-use system while providing thorough and personalized pharmaceutical care to patients within their assigned areas.
• Takes on leadership and accountability for designated clinical pharmacy sectors.
• Exhibits advanced skills and knowledge, obtaining advanced certification in their specific area of practice when available.
• Oversees patient drug therapy in alignment with relevant clinical practice standards.
• Confirms the appropriateness of all provider medication orders to maintain a high standard of patient care and safety.
• Keeps up-to-date with advanced knowledge regarding medication use, disease states, and regulations pertinent to their practice area.
• Acts as the primary source for drug information and education.
• Offers detailed patient counseling when necessary.
• Aids in the training of new staff and contributes to the development and upkeep of training materials.
• Takes local and system-wide responsibility and ownership for assigned duties.
• Follows and contributes to the creation of policies and procedures.
• Displays exceptional customer service, effective communication, decision-making, and organizational skills to ensure efficient job performance and success.
• Guarantees that adequate supplies are available for uninterrupted patient care.
• Triages and escalates any inquiries or issues beyond the scope of practice as necessary.
• Utilizes appropriate systems for processing patient information.
• Ensures quality assurance is upheld in accordance with departmental and legal standards.
• Precepts pharmacy students and/or pharmacy residents as assigned.
• Provides peer coaching and constructive feedback to colleagues in a professional and supportive manner.
• Successfully leads the local and system-wide implementation of various initiatives.
• Acts as a system expert and site lead in specialized clinical pharmacy functions.
• 3 years of experience as a Pharmacist (Required)
• Previous Clinical Pharmacy experience (Strongly Preferred)
• Bachelor's Degree in Pharmacy (Required)
• Doctorate Degree in Pharmacy (Strongly Preferred)
• Virginia Pharmacist License (Required)
• North Carolina Pharmacist License (Required/Can obtain after hired)
• Basic Life Support (BLS) certification (Required within 90 days of hire)
