• Enhance patient outcomes through telephonic interventions.
• Concentrate on the tactical implementation of medication management services, addressing issues like polypharmacy, adherence, and pharmacotherapy optimization.
• Provide counseling to patients through telephonic outreach regarding medication adherence, resolve medication-related issues such as adherence obstacles, polypharmacy, or high-risk medication usage, and offer suitable evidence-based recommendations to both patients and Practice Care Teams.
• Conduct targeted clinical chart reviews as necessary to guide patient interventions.
• Secure and document patient commitment to medication adherence while addressing barriers in real-time.
• Refer complex clinical cases to prescribers or interdisciplinary teams as needed.
• Achieve and maintain performance targets (e.g., STARs, HEDIS).
• Collaborate with Physicians, clinical, and administrative personnel to enhance the Medication Management workflow and patient experience.
• Actively identify medication management gaps through patient interactions and convert patient insights into focused recommendations aimed at improving practice operations and clinical workflows.
• Engage in the design, implementation, and evaluation of pilot programs or process modifications (“test and learn”) within the outreach framework.
• Act as a clinical subject matter expert, utilizing clinical pharmacy knowledge to support organizational goals and adapt to the changing landscape of healthcare needs.
• Create and maintain standardized resources, educational materials, and clinical protocols to foster enhanced medication outcomes and performance on quality measures (e.g., STARS, HEDIS) for practices, clinicians, and patients.
• Doctor of Pharmacy (PharmD) degree.
• Active, unrestricted pharmacist license in the state of practice; valid multi-state licensure in at least two of the following states: AL, CA, DE, KY, LA, MS, NC, OH, PA, UT, VA.
• Willingness to obtain and maintain additional licenses within six months of hire.
• Minimum of three years of experience in value-based care, ambulatory care, or community pharmacy settings.
• Extensive knowledge of pharmacotherapy and pharmacy practice.
• Excellent verbal and written communication skills; ability to convey complex clinical data in a clear and understandable manner for patients and care teams.
• Proficient in telephony platforms and various Electronic Health Record (EHR) systems.
• Capability to thrive in a fast-paced environment and adapt swiftly to changes.
• Availability to work between 8 AM - 7 PM EST as required by business needs.
