• Facilitate community well-being by enhancing medication management.
• Collaborate closely with the care coordination team and healthcare providers.
• Ensure that the assigned patients are receiving the intended prescribed therapies.
• Conduct telephonic follow-ups with members for medication reconciliation and management.
• Partner with members, physicians, health systems, and pharmacy services to address individual needs.
• Oversee transitional management, including acquiring medication orders and completing medication reconciliation.
• Assist in achieving member education objectives concerning specific chronic conditions as outlined in the Plan of Care.
• Maintain clear and concise documentation in Care Management programs.
• Doctor of Pharmacy (PharmD).
• Valid Texas state pharmacy license and/or relevant market license.
• Active residential state license in good standing.
• Clinically required licenses in good standing.
• Required licenses for Pharmacist in Charge (PIC) in good standing.
• Excellent patient engagement skills, including active listening, de-escalation intervention training, and the ability to explain complex topics clearly.
• Strong organizational and time management abilities, capable of managing multiple projects with minimal supervision.
• Outstanding written and verbal communication skills, particularly the capacity to communicate effectively and professionally over the phone.
• Proficient in computer applications such as Word and Excel.
