Clinical Pharmacist, Multidisciplinary Inbasket Support Team

This is a fully remote position, open to applicants in California.

📋 Description

• Operates both independently and collaboratively with physicians and other healthcare professionals throughout the healthcare continuum.

• Guarantees suitable medication therapy for individual patients within the Sutter system.

• Conducts refill evaluations remotely for physicians and care team in-baskets.

• Prescribes medications for refills and adjusts medications as necessary for chronic disease conditions under a collaborative practice agreement.

• Accountable for providing clinical evaluations concerning patient home readings and assisting providers with medication inquiries.

⛳️ Requirements

• PHARMD - Graduate from an accredited pharmacy school.

• PHARMR - Current registration or Registered Pharmacist within 120 days.

• ASHP - Post Graduate Year 1 ASHP General Pharmacy Residency Board Certification (preferred).

• Advanced Practice Pharmacist licensure (preferred).

• 3 years of recent relevant experience.

• Experience practicing autonomously under a collaborative practice agreement (preferred).

• Proficient in prescribing and ordering laboratory tests.

• Familiarity with medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations, and applicable laws and regulations.

• Understanding of various Pharmacy practice environments.

• Strong time management and organizational skills.
