• Deliver comprehensive support to both patients and healthcare providers.
• Cultivate personalized relationships with patients and prescribers to enhance medication adherence and advocate for evidence-based treatment.
• Assess and interpret clinicians' patient care notes and evaluations to gather necessary clinical data for prior authorization approvals.
• Create and enhance disease-specific and drug-specific clinical protocols.
• Track ongoing patient adherence across various sites and disease categories.
• Suggest alternative therapeutic drug regimens and disease-specific treatments.
• Execute clinical interventions, including thorough pharmacist consultations, motivational interviewing/coaching, and medication reconciliation.
• Identify and resolve issues related to insurance.
• Candidates must possess a Doctor of Pharmacy (PharmD) or an equivalent advanced degree and hold a valid Pharmacist license.
• A minimum of 1 year of experience is highly preferred, though not mandatory.
• Must demonstrate the ability to recognize barriers to medication adherence and develop as well as monitor strategies to enhance adherence.
• Familiarity with EPIC/QS1/WILLOW software is an advantage.
