Clinical Pharmacist, Consultation

This is a fully remote position, open to applicants in Alaska, +19 more states.

📋 Description

• Deliver patient education via telephonic consultations for patients of the specialty pharmacy, following Lumicera’s Patient Management Program.

• Address clinical inquiries from patients and caregivers.

• Maintain comprehensive clinical documentation of patient consultations and interventions using the pharmacy software system, adhering to internal policies and external regulatory standards (such as URAC).

• Analyze and evaluate prescriptions for specialty medications based on available resources and information.

• Reach out to providers and/or patients to confirm details related to prescriptions, including drug strength, intended use, and diagnosis, to expedite order processing.

• Validate and confirm the legality of controlled substances.

• Collaborate with physicians to switch prescriptions to generic, biosimilar, or preferred medications whenever feasible.

• Record and report medication errors in accordance with company policies and procedures.

• Promote error reduction and prevention strategies based on quality department and ISMP standards.

⛳️ Requirements

• Graduate from an ACPE Accredited School of Pharmacy with either a B.S. Pharmacy or PharmD degree.

• Possess a current, unrestricted pharmacist license in the state of residence; must consistently obtain educational credits to maintain active licensure.

• Preferred: A minimum of two years of experience as a practicing licensed Pharmacist.

• A strong clinical background is essential.

• Desirable: Experience or a solid understanding of Specialty Pharmacy medications.
