• Engage in pharmacist-led clinical initiatives centered on chronic disease management, quality enhancement, and medication optimization.
• Activate provided lists of eligible patients by reaching out, enrolling, and maintaining their engagement through education, outreach, and follow-up.
• Perform Medication Reconciliation, Medication Therapy Management (MTM), and Comprehensive Medication Reviews (CMR).
• Deliver telephonic patient education regarding medication usage, adherence, self-management, and escalation of care.
• Offer actionable medication recommendations and collaborate with healthcare providers through technology and direct communication to facilitate practice adoption and medication management.
• Assist other clinical programs by providing periodic consultations on pharmacy-related issues impacting patients.
• Promote medication adherence to bridge quality gaps.
• Collaborate with the Interdisciplinary Team (IDT)—comprising primary care physicians, health coaches, and nursing staff—to formulate patient-specific clinical action plans, reconcile medications, and implement necessary adjustments.
• Propel the adoption of clinical insights among providers by increasing the uptake of actionable drug-related recommendations.
• Provide clinical feedback to improve decision support logic, usability, and overall impact.
• Work in partnership with Regional Medical Officers, Market Medical Directors, Practice Transformation Managers, care teams, and practices.
• Ensure consistent clinical standards, workflows, and messaging across different markets.
• Contribute to the ideation and development of initiatives related to specialty medications, high-cost pharmaceuticals, and pharmacy optimization programs.
• Collaborate with pharmacy and medical leadership on drug strategies aligned with value-based care objectives.
• Accurately document all clinical activities in compliance with state and federal regulations and HIPAA requirements, utilizing designated platforms and technologies.
• Act as a pharmacy information resource for healthcare providers and internal clinical teams.
• Stay updated on clinical guidelines, quality measures, and pharmacy best practices.
• Doctor of Pharmacy (PharmD) degree.
• Active pharmacist license in good standing (state-specific: KS, OH, NY, NJ, FL, MD, NH, MS, LA).
• Experience in Medication Therapy Management and pharmacist-led clinical programs.
• Strong knowledge of evidence-based guidelines and quality measures.
• Experience or interest in value-based care, population health, or ACO environments.
