Clinical Pharmacist

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Manage therapy practices in line with organizational policies and procedures, while identifying and resolving medication errors and adverse reactions.

• Promptly complete necessary documentation in accordance with Client policies and protocols.

• Ensure compliance with established standards, client policies, and Visante procedures.

• Deliver suitable medication therapy and confidentially address patient inquiries.

• Collaborate with interdisciplinary teams to enhance patient care and contribute to program development.

⛳️ Requirements

• Bachelor's degree in Pharmacy or Pharm.D. from an ACPE accredited school of pharmacy.

• Current Pharmacist license issued by the relevant state licensing authority, to be obtained within 2 months of the hire date.

• American Heart Association BLS certification to be acquired within 6 months of the hire date.

• A minimum of 3 years of experience in retail or specialty pharmacy is preferred.

• Solid understanding of pathophysiology and pharmacotherapeutics.
