This is a fully remote position, open to applicants in United States.
• Evaluate medication utilization data to pinpoint trends in polypharmacy, adherence levels, and high-cost medication usage.
• Formulate and execute strategies aimed at minimizing unnecessary polypharmacy, enhancing adherence, and optimizing cost-efficient therapies.
• Partner with care teams to create interventions that decrease overall care costs while preserving or improving health outcomes.
• Track and communicate the effects of medication management strategies on both clinical and financial outcomes.
• Provide actionable insights that bolster value-based contracts and initiatives for performance enhancement.
• Engage in projects aimed at improving population health metrics associated with medication usage.
• Conduct thorough medication reviews for patients with chronic illnesses (e.g., diabetes, hypertension, obesity, heart failure, asthma, depression).
• Identify and address drug therapy issues; optimize treatment regimens; monitor and report outcomes and safety metrics.
• Counsel providers on best practices, clinical guidelines, and emerging therapies.
• Instruct patients on proper medication usage, potential side effects, and adherence strategies.
• Record all interventions, recommendations, and patient education in the electronic health record (EHR).
• Graduate from an ACPE-accredited College of Pharmacy; PharmD is mandatory.
• Successful completion of an ASHP-accredited PGY-1 Pharmacy Residency (PGY-2 preferred).
• Active pharmacy license with no state restrictions.
• A minimum of 3 years of clinical pharmacy experience in ambulatory care, preferably in a value-based care setting (CIN or ACO).
• Expertise in contemporary clinical pharmacy practices, medication therapy management, and evidence-based medicine.
• Experienced in analyzing medication data, implementing cost-containment strategies, and enhancing quality through analytics tools.
• Comprehensive understanding of the risks associated with polypharmacy, adherence strategies, and cost-effective prescribing practices.
• Proficient in medical/pharmacy terminology, computer systems, and EHR documentation.
• Capable of working effectively under pressure and collaborating within a virtual, multidisciplinary team environment.
