• Deliver comprehensive support to both patients and healthcare providers through a proactive "hands-on" approach aimed at enhancing the quality of patient care.
• Analyze and interpret clinicians' patient care notes and assessments to gather essential clinical data required for prior authorization approvals.
• Create and refine disease and medication-specific clinical protocols to optimize patient adherence and encourage evidence-based, cost-effective care.
• Track ongoing patient adherence across various sites and disease categories.
• Assess electronic medical records (EMR) to review drug and disease interactions, medication choices, lab results/vitals, allergies, and prescription orders/renewals.
• Suggest alternative therapeutic drug regimens in accordance with established guidelines when appropriate.
• Monitor and assess treatment for any side effects or other medication-related concerns.
• Execute medication therapy management (MTM).
• Implement clinical interventions, including in-depth pharmacist consultations, motivational interviewing/coaching, drug regimen optimization, and medication reconciliation.
• Track productivity and provide weekly statistics for growth development.
• Supervise the daily activities of technicians and ensure accuracy in delivery coordination slips and submitted prior authorizations.
• Identify and address insurance-related issues, including recognizing opportunities for financial assistance.
• Oversee communication among support personnel in compliance with HIPAA regulations.
• Maintain current registration and ensure all remote pharmacist activities adhere to applicable regulations.
• Engage in direct communication with other healthcare professionals concerning patient care.
• Required Licensure/Certification: Candidates must possess a Doctor of Pharmacy (PharmD) or an equivalent advanced degree and be a licensed Pharmacist in the state of Oregon.
• A minimum of 1 year of experience in a clinical role is highly preferred but not mandatory.
