Clinical Pharmacist

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Deliver comprehensive support to both patients and healthcare providers.

• Cultivate personalized relationships to enhance medication compliance.

• Analyze and interpret notes from clinicians regarding patient care.

• Create and improve disease- and drug-specific clinical protocols.

• Track patient adherence across various disease categories.

• Assess electronic medical records (EMR) for medication adherence and enhance outcomes.

• Suggest alternative therapeutic drug regimens.

• Monitor and assess therapies for potential side effects.

• Perform medication therapy management (MTM).

• Execute clinical interventions and quality improvement initiatives.

• Ensure compliance with local, state, and federal drug regulations.

• Oversee support staff while adhering to HIPAA regulations.

• Engage in educational programs and provide training for pharmacy personnel.

⛳️ Requirements

• Candidates must possess a Doctor of Pharmacy (PharmD) or an equivalent advanced degree.

• A minimum of 1 year of experience is preferred but not mandatory.

• Familiarity with EPIC/QS1/WILLOW software is advantageous.

• In-depth understanding of specialty medication pharmacology is essential.

• Excellent verbal and written communication skills are a must.

• Proficiency in Microsoft Word, Excel, and PowerPoint is required.

• Ability to recognize barriers to medication adherence.

• Skills in motivating and coaching patients to achieve improved outcomes.
