• Deliver disease and medication management via telephone and virtual consultations.
• Conduct thorough medication reviews and assist with chronic disease management.
• Identify and address medication-related issues to improve safety and efficacy.
• Suggest evidence-based therapeutic alternatives and collaborate with healthcare teams.
• Accurately document all patient interactions within our clinical care platform.
• Facilitate transitions of care through medication reconciliation and follow-up.
• Educate patients and their caregivers on medication adherence and health literacy.
• Stay informed about emerging best practices and regulatory standards.
• Provide clinical insights and leadership to internal stakeholders.
• Engage in continuous performance improvement and outcome reporting.
• PharmD degree from an ACPE-accredited institution.
• Licensed or eligible for pharmacist licensure in Virginia.
• Board Certification (BCGP, BCPS, BCACP) preferred or achievable within 12 months.
• Excellent clinical reasoning and patient-focused communication abilities.
• Familiarity with chronic disease management and value-based care principles.
• Proficient in Google Workspace and clinical software for documentation.
• Self-driven, organized, and able to adapt to a virtual care setting.
