• Provide disease and medication management services through phone or telehealth (video) platforms.
• Perform thorough medication reviews to pinpoint therapy gaps and medication-related concerns.
• Oversee drug therapy to check for contraindications, interactions between drugs or food, and dosing precision.
• Suggest cost-effective and clinically suitable alternatives to the medical staff.
• Work in close collaboration with internal care teams and external partners to ensure coordinated patient care.
• Record all clinical and care management activities in PharmD Live’s proprietary system.
• Educate patients regarding medication usage, adherence strategies, and treatment objectives.
• Contribute to quality initiatives by providing insights and supporting enhancements to programs.
• Pursue continuous professional development through ongoing education and research.
• Offer clinical oversight and support to guarantee timely and value-driven delivery to clients.
• Present updates and progress reports to leadership as required.
• PharmD degree from an ACPE-accredited College of Pharmacy.
• Active Minnesota pharmacist license or eligibility for licensure.
• Board Certification (BCGP, BCPS, BCACP) or a commitment to obtain it within 12 months.
• Proven ability to work independently and exhibit sound clinical judgment.
• Up-to-date knowledge of all aspects of pharmacy practice.
• Excellent communication, documentation, and analytical abilities.
• Capacity to handle multiple priorities in a fast-paced care environment.
• Proficiency in Google Workspace and Windows operating systems.
