Clinical Pharmacist

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Assist in the development and execution of Monogram's medication therapy management (MTM) program.

• Conduct timely written medication reconciliations and create drug therapy action plans for all Monogram members.

• Engage in telephonic consultations with healthcare providers.

• Collaborate effectively with clinical care manager operations personnel.

• Track significant medication availability and usage trends for populations under Monogram's care management.

• Remain updated on evidence-based prescribing practices for individuals with kidney disease.

• Take part in interdisciplinary care team case rounds or meetings.

⛳️ Requirements

• Bachelor’s degree in pharmacy or PharmD.

• Valid pharmacist license.

• A minimum of 3 years of relevant experience (or equivalent) actively practicing as a pharmacist, ideally within a managed care, ambulatory care, or clinical practice setting.
