Clinical Pharmacist, Advanced Therapeutics/Biologics – Dermatology

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Delivers patient-focused clinical pharmacy services, encompassing comprehensive medication reviews, adherence monitoring interventions, and targeted medication assessments aimed at enhancing clinical, economic, and patient outcomes.

• Develops and executes targeted medication and disease management initiatives.

• Tasked with enhancing the patient’s experience, ensuring safety, and achieving favorable clinical and financial results.

• Records clinical interventions and activities promptly.

• Maintains precise and appropriate patient medication profiles, overseeing the suitability of medication choices, dosage forms, strengths, therapy durations, and potential interactions of prescribed medications.

• May operate under Collaborative Practice agreements and protocols as sanctioned by healthcare providers.

• Oversees the activities of pharmacy technicians involved in Medication Therapy Management (MTM) and transitions of care.

• Guarantees the timely and accurate dispensing of medications from the pharmacy area.

• Accountable for the precise filling and dispensing of medications from the pharmacy area.

• Advocates for the utilization of formulary drugs by converting non-formulary orders to formulary alternatives wherever feasible, in accordance with P&T protocols.

• Identifies, monitors, documents, and reports adverse drug reactions and medication errors.

⛳️ Requirements

• Required: Doctor of Pharmacy (Pharm.D) degree; OR Bachelor of Science in Pharmacy (BSPharm) degree with 3 years of hospital experience.

• Licensure with the Massachusetts Board of Registration in Pharmacy is mandatory.

• Preferred: ASHP Accredited Residency in Pharmacy Practice (PGY1) is strongly recommended.

• Pharm.D. degree with at least 1 year of hospital experience is desirable.

• Alternatively, unless certification, licensure, or registration is mandated, an equivalent combination of education and experience that demonstrates proficiency in the responsibilities outlined in this description may be accepted in lieu of the stated requirements.
