Clinical Nurse – Call Center

This is a fully remote position, open to applicants in United States.

📋 Description

• Reach out to patients promptly based on alerts, established protocols, or follow-up requirements.

• Perform telephonic evaluations, assess findings that necessitate provider intervention, pinpoint educational or coaching needs, and promote best practices in foot care.

• Deliver a patient-focused approach that ensures an outstanding patient experience.

• Assist in patient compliance within the Podimetrics program.

• Provide insights and feedback regarding patient experience to improve programs, processes, and technology.

• Record patient findings, symptoms, and trends utilizing Podimetrics systems and standard operating procedures.

• Recognize high-risk indicators and initiate clinical escalations to providers or care teams.

• Oversee assigned queues, which include outbound calls, inbound calls, escalations, inflammation check-ins, and pilot-specific protocols.

• Ensure documentation is completed within specified timelines.

• Maintain dependable remote connectivity and independently resolve basic technical issues.

• Collaborate with peers, leads, and managers to ensure continuity of care and smooth transitions.

• Share insights during team huddles and meetings.

• Achieve quality metrics, productivity targets, and patient experience expectations.

• Engage in peer evaluations, audits, and improvement initiatives.

⛳️ Requirements

• Active and unrestricted Registered Nurse (RN) compact license.

• Bilingual skills (Spanish or another language).

• At least 2 years of clinical nursing experience.

• Experience in a fast-paced work environment.

• Strong clinical judgment, telephone etiquette, and attention to documentation accuracy.

• Proficient in navigating electronic health tools, workflows, and productivity platforms.

• Self-motivated with excellent time management skills and adaptability in a remote work environment.

• Preferred experience in telehealth, home health, or remote monitoring.

• Preferred background in chronic disease management (diabetes, CHF, wound care, etc.).

• Familiarity with CMS quality metrics and telehealth compliance is preferred.

• Ability to work during PST hours is preferred.
