Clinical MTM Pharmacist

This is a fully remote position, open to applicants in United States.

📋 Description

• Offer patient-focused Medication Therapy Management (MTM) services to enhance medication treatment.

• Conduct Comprehensive Medication Reviews, Disease-Specific Medication Reviews, and Targeted Interventions.

• Effectively communicate and document clinical recommendations to healthcare providers.

⛳️ Requirements

• At least 1 year of experience in a clinical pharmacy setting.

• Bachelor’s Degree in Pharmacy or PharmD from an ACPE accredited institution, or a graduate from an international college of pharmacy.

• Valid pharmacist licensure.

• Willingness to obtain pharmacist licensure in multiple states if necessary.
