Clinical Manager – Skilled Home Health

This is a fully remote position, open to applicants in Florida.

📋 Description

• Assess new referrals, verify necessary disciplines, align care plans and objectives, and update alerts/journal entries.

• Monitor cases through audits and case discussions; remain available during operating hours for clinician inquiries.

• Conduct ride-alongs and provide field coaching for onboarding, performance improvement, or quality enhancement initiatives.

• Eliminate obstacles to timely documentation, enforce standard operating procedures, and ensure OASIS and care plans accurately reflect current status.

• Encourage appropriate use of Telehealth and the Patient Portal.

• Hold regular one-on-one meetings with assigned staff and the Clinical Assistant.

• Reinforce standards, issue corrective actions when necessary, and promote ongoing competency.

• Perform quarterly record reviews and communicate findings and recommendations to DCS/ADCS.

• Aid in branch quality improvement initiatives.

• Assist in screening and interviewing candidates, recommending hires, and implementing onboarding checklists and early-career coaching.

• Lead in-service sessions and designated educational activities; monitor completion and retention of essential practices.

• Exemplify the mission and values while maintaining a respectful, compliant atmosphere.

• Execute additional tasks assigned by DCS/ADCS/Regional Leadership to fulfill patient care and operational requirements.

⛳️ Requirements

• Must possess a current RN license (specific to the state).

• At least two years of experience in home health or a related field.

• Strong leadership and management capabilities.

• Exceptional communication, organizational, and time management skills.

• Familiarity with healthcare regulations and home health standards of care.

• Proficient in utilizing electronic medical records (EMR) and other healthcare technologies.
