This is a fully remote position, open to applicants in Arizona, +10 more states.
• Engage in outreach initiatives with newly referred clients and record activities in Salesforce.
• Conduct 8-12 intake visits each day, ensuring assessments are both efficient and accurate.
• Support clients during initial intake visits, which include psychosocial assessments and the development of care plans with specific goals.
• Review medical records and document clients' medical requirements along with recommended interventions.
• Relay essential information and client preferences to the care team for tailored care planning.
• Apply evidence-based methodologies such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Participate in weekly Team Operations Meetings.
• Keep a well-organized schedule to maximize client intake and engagement.
• Conduct proactive outreach to clients who have missed appointments or have not yet been contacted when unscheduled.
• Be readily available for warm transfers from field teams and ensure same-day responses.
• Achieve key performance metrics in a fast-paced and dynamic setting.
• A valid and unrestricted California Licensed Vocational Nurse (LVN) license, or a Bachelor’s degree in a relevant field, or a Medical Assistant certificate from an accredited organization (AAH, AAMA, AMT, NCCT, or NHA).
• At least 2 years of prior experience in intake or admissions within a high call volume environment.
• Familiarity with complex care clients and ECM Populations of Focus.
• Proficient in Health Gorilla or other Health Record Systems.
• Excellent verbal and written communication skills.
• Strong interpersonal skills with a cooperative approach.
• Advanced computer skills and proficiency in Google Workspace, MS Office, and internet tools.
• Capability to work independently and maintain high motivation levels.
• Ability to juggle multiple tasks, navigate various platforms, and adjust priorities as needed.
• Bilingual in English and Spanish is highly preferred.
• Availability to work Monday through Friday, from 8:30am to 5pm Pacific Time, with potential evening and weekend shifts.
• A minimum of 1 year of remote (work-from-home) experience.
• Comfort with using computers, telephones, and electronic health record (EHR) systems.
