This is a fully remote position, open to applicants in Canada.

📋 Description

• Serve as a bridge between long-term care facilities, retirement communities, and CareRx Pharmacy to guarantee superior Resident care, Pharmacy Services, and Clinical support.

• Travel to various LTC homes and retirement communities.

• Develop and sustain relationships within the community to ensure continuous collaboration and communication between leadership teams of homes/communities and pharmacy teams to cultivate strong partnerships.

• Perform medication reviews in retirement communities and long-term care homes currently serviced by CareRx Pharmacy.

• Complete all necessary documentation and billing processes as mandated by MOHLTC, College of Pharmacists of British Columbia, and other federal and provincial standards and regulations.

• Provide educational resources to Residents and staff on various health-related subjects, including the Meds Check program, and participate in community-based wellness fairs and other events as needed.

• Conduct educational sessions within Long Term Care Homes and Retirement Communities within the scope of practice, as applicable (e.g., policy and procedure, web portal, eMAR, influenza protocol, etc.).

• Uphold CareRx Pharmacy policies and procedures within retirement communities and long-term care homes.

• Conduct medication audits on-site in LTC Homes and Retirement Communities as required.

• Ensure regular on-site visits are made to all assigned clients.

• Attend and engage in annual professional conferences and trade shows, when applicable.

• Contribute to the development and implementation of industry-leading clinical pharmacy services, when applicable.

• Serve as a resource for education, policy, and pharmacy initiatives.

• Be available for special clinical team projects as required.

• Collaborate in the development of medication audit follow-up improvement plans when applicable.

• Work cooperatively with other team members to support relevant homes and communities.

• Participate in pharmacy-related transition processes (e.g., new client transitions, eMAR transitions, etc.).

• Assist with pharmacy operations and drug distribution as needed.

• Attend and actively engage in clinical team meetings.


⛳️ Requirements

• Insured and currently licensed in good standing with the College of Pharmacists of British Columbia.

• Demonstrated ability to cultivate strong partnerships with other members of the healthcare team.

• Effective organizational, planning, and time management skills.

• Proficient in Kroll Windows application and Microsoft Office programs.

• Comprehensive understanding of provincial regulations governing LTC homes and Retirement Communities.

• Familiarity with the provincial standards of practice established for LTC pharmacies.

• Outstanding customer service and interpersonal skills, along with effective oral and written communication skills, able to interact with individuals across various levels of professional expertise.

• Excellent time management capabilities.

• Skilled in critical thinking processes and able to prioritize tasks effectively.

• Must exhibit initiative and creativity.

• Self-directed, requiring minimal supervision, and capable of working independently.

• Driven to achieve results that reflect a high standard of quality.

• Strong listening skills and adaptable to changes in the work environment.

• Reliable and responsible in the performance of job duties.

• Flexible to changes within internal work structures.

• Strong presentation and teaching abilities.

• Must possess a valid driver’s license.
