Clinical Chart Abstractor, LPN/RN Required

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate patient EMR records for cardiac recovery patients to establish medical eligibility or ineligibility for Aviary Health.

• Offer daily administrative assistance to the enrollment team by analyzing patient charts.

• Identify existing cardiac rehab referral orders or generate new orders when necessary.

• Convey patient eligibility and ineligibility to the enrollment team through precise documentation on the appropriate platforms.

• Review clinical inquiries submitted by cardiac therapists in SBAR format, ensuring all required information is included.

• Evaluate clinical scenarios using established Exercise Protocols and ClearTriage guidelines.

• Provide timely, protocol-compliant clinical guidance to cardiac therapists.

• Act as the primary clinical resource for Care Team members by monitoring and addressing real-time clinical inquiries.

• Complete special projects as required.

• Acquire and maintain a comprehensive understanding of examinations, making swift, independent decisions using sound judgment.

⛳️ Requirements

• 3-5 years of clinical experience in cardiac care, exercise physiology support, or chronic disease management.

• 3 years of experience with EPIC or general EMR systems.

• 1 year of experience with medical terminology.

• Proficient in computer applications, including Zoom, Google Suite, and EMR systems.

• Excellent written and verbal communication abilities.

• Active RN or LPN license required.

• Experience with ClearTriage or comparable telephone/virtual triage systems.

• Strong clinical assessment, triage, and analytical competencies.

• Familiarity with Citrix.

• EPIC access RTE required.

• Competent in Google Suite.

• Experience with Zoom.

• Proficient in Teams, necessary for GHS Epic training.

• Reliable internet connection and a quiet workspace suitable for participating in live patient sessions.

• Availability Monday-Friday for 3-5 hours per day / 15-20 hours per week within the 3pm-9pm ET timeframe.

• Availability for eligibility/enrollment team meetings.
