• Evaluate the needs of members in areas such as physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referrals to community-based services and supports.
• Supervise the creation of client care plans and the setting of goals.
• Provide services at the member's location, whether that be in-person, through telehealth, or in the field, ensuring accessibility.
• Link clients to additional social services and supports as required.
• Advocate for the client with healthcare professionals (e.g., primary care providers).
• Implement evidence-based methodologies, including Motivational Interviewing, Harm Reduction, and principles of Trauma-Informed Care.
• Perform outreach and engagement efforts to facilitate access to the ECM program and document activities in the Client Relationship Management (CRM) system.
• Assess client progress and revise SMART goals accordingly.
• Promote mental health awareness and support.
• Coordinate transportation services (e.g., ACCESS).
• Ensure all documentation, including outcome measures, is completed within the timelines set by individual care plans.
• Keep patient health records current in the Electronic Medical Record (EMR) system and other relevant business systems.
• Submit monthly reports to maintain program compliance.
• Participate in assigned training sessions.
• Must possess an active, unrestricted LCSW, LMFT, or LPCC license in California.
• At least 1 year of experience in roles such as care manager, care navigator, or community health worker supporting at-risk populations.
• Familiarity with government and community resources related to social determinants of health is essential.
• Exceptional oral and written communication skills are required.
• Strong interpersonal skills are a must.
• Proficiency with general computer skills and a working knowledge of Google Workspace, MS Office, and the internet is necessary.
• Bilingual proficiency in English and Spanish is highly preferred.
