Clinical Care RPM LVN

This is a fully remote position, open to applicants in United States.

📋 Description

• Make outbound calls to patients for health check-ins and to address their concerns, with a target of 70 to 90 calls daily.

• Manage incoming calls, directing them based on clinical urgency.

• Forward non-clinical inbound calls to the relevant departments within the organization.

• Oversee and react to Remote Patient Monitoring (RPM) alerts, escalating issues when clinically necessary.

• Work alongside providers to ensure timely and effective patient care.

• Conduct monthly wellness evaluations and thorough chart reviews.

• Accurately log all patient interactions in the clinical platform in real-time.

• Achieve or surpass personal and team performance metrics concerning care quality, patient engagement, response times, and compliance with protocol standards.

• Adhere to company policies and relevant regulations.

• Undertake additional duties as assigned.

• Assist patients through real-time health monitoring, prompt interventions, education, and continuous care coordination.

⛳️ Requirements

• An active LVN/LPN license is mandatory.

• Proficient with computers, EMRs, and telehealth applications.

• Strong communication and organizational abilities are essential.

• A minimum of 1 year of nursing experience is preferred.

• Experience in RPM, telehealth, or chronic care is advantageous.

• A minimum internet speed of 50 Mbps download and 10 Mbps upload is required.

• A hardwired internet connection is necessary.

• Submission of a speed test is required during the offer process.

• A private workspace compliant with HIPAA regulations is essential.

• Availability to work rotating holidays throughout the year is required.

• Mandatory coverage for at least two weekends each month.

• Required attendance for a five-week training program.

• Training availability from Monday to Friday, 9:00 AM – 6:00 PM Eastern Time.

• A genuine interest in and comfort with AI tools such as Claude, ChatGPT, and workflow automation platforms.

• Ability to work a rotating schedule, including two weekends per month, from 9:00 AM – 8:00 PM EST.

• Evening Differential: +$1.50/hour for hours worked after 7:00 PM.
