This is a fully remote position, open to applicants in Maryland.
• Oversee the admission and discharge processes for care management services.
• Utilize the case management approach to direct service provision.
• Create and sustain personalized care plans that outline desired outcomes.
• Coordinate services and supports to guarantee the execution and adherence to contractual and regulatory obligations.
• Supervise ongoing services, assess utilization and cost-effectiveness, and suggest modifications to plans as necessary.
• Assess the gap between desired and actual outcomes and record progress accordingly.
• Identify and propose cost-effective care alternatives.
• Act as a clinical resource for community service providers.
• Maintain continuous communication with clients, families, and healthcare team members.
• Organize, participate in, and engage actively in multidisciplinary team meetings.
• Provide services through both in-person visits and virtual/remote interactions.
• Keep precise, timely, and comprehensive care management records.
• Record client interactions in the client database within 48 hours of contact.
• Ensure adherence to HIPAA regulations and all relevant professional standards.
• Bachelor of Science in Nursing (BSN) with an active Registered Nurse (RN) license or a Master's degree in social work (MSW) with an active LMSW or LCSW-C license.
• Degree must be obtained from an accredited college or university or equivalent education and experience.
• CCM (Certified Case Manager) certification is preferred and required within two years of employment.
• A minimum of three (3) years of professional experience in areas such as Pediatric intensive care nursing, Ambulatory health care, Rehabilitation nursing, or Clinical social work in a community-based setting with individuals facing complex healthcare needs.
• Proven clinical competence and the ability to operate independently with minimal oversight.
• Active state licensure and current malpractice insurance (to be maintained at all times).
• Maintenance of relevant professional certifications (e.g., CCM, RN, LCSW-C).
• Valid driver's license, dependable transportation, and proof of automobile insurance.
• Capability to travel throughout Maryland.
• Designated home office that complies with all HIPAA requirements.
• Ability to work remotely from a home-based office.
• High-speed internet access.
• Proficiency in Microsoft Office Suite (Word, Excel, Access, Outlook), databases, and remote communication/documentation tools.
