Clinical Care Coordinator

This is a fully remote position, open to applicants in Utah.

📋 Description

• Engage with patients via phone calls, email, SMS, and the patient portal.

• Provide a caring, professional, and patient-focused experience during every interaction.

• Adjust to changing workflows and assist with the implementation of new clinical programs as they arise.

• Work collaboratively with providers and internal teams to enhance patient care, addressing medication inquiries, side effects, dosing, and general treatment support.

• Direct and escalate clinical issues to providers when necessary.

• Oversee prescription workflows in conjunction with pharmacy and fulfillment teams.

• Accurately and efficiently record patient interactions in internal systems.

• Aid in care coordination across GLP-1, peptides, HRT, wellness initiatives, and other clinical programs as our organization expands.

• Achieve performance benchmarks for response time, quality, resolution, and patient experience.

⛳️ Requirements

• Valid LVN/LPN license or experience in a medical office.

• At least 2 years of clinical or medical experience within the past 4 years.

• A minimum of 1-2 years of remote or telehealth experience.

• Exceptional written and verbal communication abilities.

• Strong organizational skills with the capability to manage multiple tasks in a dynamic environment.

• Ability to work independently while staying actively engaged in a remote setting.

• Proficient in computer usage and quick to learn new software and workflows.

• Familiarity with GLP-1s, peptides, HRT, men's health, or other telehealth treatment programs is preferred.

• Experience with EMR, CRM, or ticketing systems is preferred.

• Availability for a Thursday–Monday work schedule is preferred.

• Experience in a fast-paced, high-volume environment is preferred.
