This is a fully remote position, open to applicants in Maryland.
• Perform patient outreach and engagement through video messaging, video consultations, phone calls, and text-based communication, educating and enrolling patients into the adherence program.
• Leverage video-based communication as a primary tool to provide personalized education, coaching, and support for adherence.
• Deliver compassionate and motivational coaching, case management, and health education services to promote medication adherence and encourage lifestyle modifications.
• Execute video-based medication reconciliation consultations, ensuring patients comprehend their medications, dosing schedules, and proper administration methods.
• Review asynchronous video submissions from patients regarding medication intake, verifying correct administration, monitoring adherence, and evaluating side effects or concerns.
• Produce short-form educational and motivational videos for personalized patient engagement and population health education.
• Evaluate patient needs, incorporating medical, behavioral, and social challenges, and escalate cases when necessary.
• Provide guidance on medication administration, inhaler techniques, chronic disease management, and medication safety through both live and asynchronous video coaching.
• Serve as a clinical escalation point for non-clinical team members to submit patient cases requiring a higher level of clinical review and intervention.
• Assist patients with technical troubleshooting related to the company’s mobile application and adherence platform.
• Keep accurate and detailed documentation of all interactions, ensuring adherence to company and regulatory standards.
• Engage with provider offices, health plan services, and pharmacies on behalf of patients as needed to help overcome barriers.
• Provide referrals to healthcare providers and community resources when necessary, including direct outreach to provider offices and/or facilitating warm transfers to service providers.
• Employ motivational interviewing techniques during video and digital engagement to encourage patients in long-term adherence strategies.
• Offer insights on inhaler techniques, chronic disease management, and medication safety.
• Contribute to program enhancements by sharing insights from patient interactions, particularly regarding video engagement, digital adherence monitoring, and educational strategies.
• Collaborate with cross-functional teams to create and refine video-based patient engagement content, clinical protocols, and educational materials.
• Bilingual in Spanish.
• Registered Nurse (RN) license in good standing with a minimum of 5 years of nursing experience.
• At least 5 years of relevant experience in patient engagement, health education, or care coordination.
• Strong expertise in chronic disease management, medication adherence, and patient coaching.
• Exceptional oral and written communication skills with a warm, empathetic demeanor.
• Ability to thrive in a fast-paced, patient-centered environment.
• Proficient in electronic health records (EHR), mobile applications, and digital health platforms.
• Comfortable engaging patients through phone, video, and chat throughout the workday.
• Experience in assessing social determinants of health and connecting patients with appropriate resources.
