This is a fully remote position, open to applicants in United States.
• Ensure the confidentiality and precision of patient information in accordance with HIPAA regulations.
• Accurately and promptly maintain medical records, ensuring they are well-organized and kept confidential.
• Address inquiries from healthcare providers related to patient records and information.
• Gather patient information from healthcare professionals and input data into EMR systems for Chronic Care Management initiatives.
• Assist in the collection, organization, and review of comprehensive Past Medical History, guaranteeing that optimal care options are available and accurately documented in patient records.
• Perform detailed reviews of patients’ health histories to identify contraindications to exercise and participation in cardiac or pulmonary rehabilitation programs following Carda Health protocols.
• Utilize knowledge of cardiac and pulmonary medications to facilitate effective medication reconciliation and identify risks that could hinder safe participation.
• Index and cross-reference files for straightforward retrieval and effective record management.
• Enter patient data into the medical records system, including updates to demographics, medical history, and treatment plans.
• Provide general administrative support and assist with various tasks to ensure smooth operations within the medical office.
• Collaborate directly with Carda’s founders and clinical team to innovate and enhance our state-of-the-art digital care delivery model.
• A Bachelor's degree in Clinical Exercise Physiology / Exercise Science.
• At least two years of experience in a medical office environment, ideally in cardiac and pulmonary rehabilitation.
• Proficient in medical records systems and relevant software.
• Exceptional attention to detail and accuracy in data entry and record management.
• Familiarity with medical terminology and a strong understanding of medical record documentation.
• Strong organizational, problem-solving, and communication abilities.
• Knowledge of HIPAA regulations and a commitment to maintaining patient privacy and confidentiality.
• Bonus Points: Over 2 years of experience in a similar role.
• Experience with remote work.
• ACSM Clinical Exercise Physiologist Certification.
• AACVPR CCRP certification.
• Registered Health Information Technician (RHIT) or a comparable credential.
