This is a fully remote position, open to applicants in California.
• Collaboratively create a comprehensive patient-centered care plan and objectives with the client/family and other healthcare providers.
• Engage and outreach to patients by working effectively with team members.
• Advocate for clients and their families to assist them in accessing entitlements and various community services.
• Collaborate closely with in-office providers and staff to manage daily communications with enrolled patients regarding symptom control, medication management, education for patients and families, health maintenance reminders, medication refills, and coordination of referrals.
• Answer and respond to incoming calls from patients, triaging and offering medical advice as directed by the patient's provider.
• Record calls following established protocols.
• Keep the patient's care team informed with updated information as it is received.
• Participate in educational and in-service programs.
• Provide on-call support for the assigned clinic.
• At least 1-2 years of experience in a clinical or telehealth environment.
• Availability to work full-time (minimum of 40 hours).
• Demonstrated ability to effectively utilize available computer technology, including proficient typing skills.
• Typing speed of 60+ words per minute.
• Required skills in customer service, leadership, communication, interpersonal interaction, and problem-solving.
• Capability to connect a computer directly to an internet router.
• Access to high-speed internet.
• Graduate from an accredited School of Nursing.
• Valid license to practice as an LVN/LPN/RN with a compact license.
