This is a fully remote position, open to applicants in United States.
• Provide chronic care management services under the guidance of the nurse practitioner for patients involved in Phase 3 Pulmonary Rehabilitation.
• Motivate patients to achieve their health objectives while ensuring their safety.
• Assess the medical suitability for exercise-based programs.
• Address patients’ urgent medical requirements.
• Uphold a high standard of care that aligns with clinical protocols.
• Safeguard the privacy and accuracy of patient information in accordance with HIPAA regulations.
• Maintain timely and accurate medical documentation, ensuring it is well-organized and confidential.
• Conduct video consultations biweekly and communicate with patients via phone, text, and email.
• Guide patients in their health pursuits, focusing on accountability, motivation, and support.
• Monitor vital signs throughout sessions and manage live emergencies as necessary.
• Coordinate patient care by scheduling appointments and assisting with medication clarifications or adjustments from healthcare providers.
• Deliver personalized education on diseases, therapies, medications, and available patient resources.
• Provide general administrative assistance and support other tasks to ensure efficient medical-office operations.
• Collaborate directly with Carda’s founders and clinical team to enhance and innovate the digital care delivery model.
• Bachelor’s degree in Clinical Exercise Physiology or Exercise Science.
• At least two years of experience in a medical office environment, ideally in cardiac and pulmonary rehabilitation.
• Familiarity with medical records systems and software applications.
• Exceptional attention to detail and accuracy in data entry and record management.
• Understanding of medical terminology and proficiency in medical record documentation.
• Strong organizational, problem-solving, and communication abilities.
• Knowledge of HIPAA regulations and a strong commitment to patient privacy and confidentiality.
• Experience in providing patient care within a clinical setting.
• Ability to remain composed in emergency situations.
• Experience with live video telehealth consultations.
• Comprehensive knowledge of common disease processes such as COPD, CAD, and T2DM.
• Familiarity with the therapies and medications associated with these conditions.
• Comfortable using various technology platforms and adaptable to rapid changes in a clinical environment.
