This is a fully remote position, open to applicants in United States.
• Operate autonomously while managing a monthly roster of patients.
• Enhance HealthXL's CCM/RPM platform to ensure services are delivered consistently and in compliance with regulations.
• Make several attempts each month to contact every patient and create/update their personalized Care Plan.
• Ensure computer access is HIPAA compliant.
• Adhere to iCare standard workflows and questionnaires to maximize patient engagement and documentation.
• Conduct monthly telephone interactions with patients to assess their health status and promote adherence to medication/treatment plans.
• Facilitate the communication of essential information to the practice on behalf of patients.
• Reside in the United States.
• Fluent in both Chinese and English.
• Possess credentials (Certified/Registered Medical Assistant, LPN/LVN, RN/BSN).
• Experience with Chronic Care Management and/or Remote Patient Monitoring is advantageous.
• Comfortable with setting up, accessing, and troubleshooting provided equipment and technology issues.
• Dedicated to upholding HealthXL's values of integrity and excellence.
• At least 1 year of experience in a clinical or patient-focused environment.
• Strong critical thinking abilities.
• Organized with meticulous documentation skills.
• Self-motivated with the capacity to prioritize tasks and manage time effectively.
