
Chart Scrub Virtual Assistant – Care Coordinator

This is a fully remote position, open to applicants in Philippines.
• Review assigned patient charts utilizing approved chart-scrubbing workflows.
• Verify patient status, provider assignments, contact details, care settings, and the most recent visit information.
• Identify overdue or impending follow-up recommendations, missed appointments, incomplete referrals, unresolved orders, absent documentation, and unsuccessful outreach attempts.
• Classify charts and direct patients to the appropriate scheduling, clinical, or administrative queues.
• Provide reasons for visits and suggest follow-up timeframes as needed.
• Reach out to patients, caregivers, facilities, or authorized representatives following established procedures.
• Confirm patient identity and articulate recommended follow-up using approved language.
• Schedule both routine and priority follow-up appointments.
• Confirm appointment specifics and identify any barriers to care.
• Promptly direct urgent symptoms, abnormal findings, safety issues, medication inquiries, worsening symptoms, and treatment concerns to licensed clinical staff.
• Adhere to emergency escalation protocols for potentially life-threatening symptoms.
• Complete standardized chart-review notes and document visits, recommendations, events, concerns, outreach, routing decisions, and outcomes.
• Maintain work queues, patient trackers, and follow-up lists.
• Engage in quality reviews and rectify any documentation or routing errors.
• Safeguard patient information and comply with HIPAA, company, and client confidentiality standards.
• Operate within established client workflows and under designated clinical or administrative leadership.
• Minimum of one year of experience in healthcare administration, medical scheduling, patient coordination, care management, or a related healthcare position.
• Proficiency in reviewing electronic health records and navigating patient charts.
• Knowledge of medical terminology, provider orders, laboratory reports, medication lists, and healthcare documentation.
• Strong written and verbal communication skills in English.
• Exceptional attention to detail and accuracy in data entry.
• Capability to identify missing, inconsistent, or unresolved information.
• Ability to differentiate between administrative needs and issues requiring licensed clinical review.
• Strong organizational, time-management, and follow-up abilities.
• Skill in managing multiple patient records and deadlines with precision.
• Proficiency in communicating professionally and compassionately with patients, caregivers, providers, and facilities.
• Ability to work independently while adhering to established workflows and escalation procedures.
• Willingness to work during U.S. business hours or night shifts in the Philippines.
• Preferred: Philippine Registered Nurse or Licensed Nurse.
• Preferred: Graduate in Nursing or another allied health or medical field.
• Preferred: Licensed pharmacist, medical technologist, physical therapist, or another related healthcare professional.
• Preferred: Experience as a medical assistant, nursing assistant, care coordinator, or health unit coordinator.
• Preferred: Two or more years of experience in primary care, chronic care management, population health, home health, hospice, or care coordination.
• Preferred: Experience supporting Medicare patients, older adults, or patients with multiple chronic conditions.
• Preferred: Experience coordinating care with assisted living facilities, group homes, skilled nursing facilities, caregivers, and specialists.
• Preferred: Proficiency in using electronic health records, patient portals, healthcare management systems, or telephone outreach platforms.
• Preferred: Familiarity with Homecare Homebase, WellSky, AxisCare, Axxess, AlayaCare, MatrixCare, or similar platforms.
• Preferred: Understanding of HIPAA, patient confidentiality, and healthcare documentation regulations.
• Preferred: Previous experience as a remote healthcare Virtual Assistant.
