
Certified Medical Assistant β Primary Care Coordinator

This is a fully remote position, open to applicants in Arizona.
β’ Act as the main liaison for patients, families, facility partners, and providers within your designated patient panel.
β’ Assess and prioritize urgent care requests, STAT tasks, and critical clinical communications in real-time.
β’ Ensure precise and compliant patient documentation is maintained within our EMR system.
β’ Actively manage appointment scheduling, follow-up care, and interdisciplinary communication.
β’ Oversee the onboarding process for new patients, guaranteeing each patient is contacted and scheduled within 24 hours of enrollment.
β’ Join after-hours on-call rotations to ensure continuity of care.
β’ Participate in quality improvement initiatives and projects aimed at enhancing operational efficiency.
β’ At least 2 years of direct patient care experience in a clinical environment (such as Medical Assistant, LVN/LPN, CNA, Phlebotomist, Patient Care Technician, or a comparable role).
β’ Background in healthcare coordination, patient services, or clinical operations.
β’ Experience with geriatric, Medicare Advantage, or value-based care is highly preferred.
β’ Strong skills in triage and prioritization in high-volume, real-time settings.
β’ Proficiency in EMR/EHR systems and healthcare technology platforms.
β’ Outstanding communication abilities with a diverse range of stakeholders (patients, families, physicians, facility leadership).
β’ Self-motivated and responsible, capable of working independently in a remote environment.
β’ Availability for on-call shifts: approximately 5 overnight and/or weekend shifts per rotation.
β’ Remote work requirements include reliable high-speed internet and a dedicated, HIPAA-compliant workspace.
β’ Must adhere to regulatory compliance, maintaining good standing with OIG, CMS, and Federal Health Care Programs (no exclusions or debarments).
