Centralized Triage Registered Nurse, RN

This is a fully remote position, open to applicants in Alabama, +2 more states.

📋 Description

• Act as the main contact for hospice patients and their families during after-hours, weekends, and holidays.

• Triage incoming calls, evaluate patient needs, and deliver clinical advice and interventions over the phone.

• Address or redirect non-clinical inquiries related to durable medical equipment, pharmacy, scheduling, and general inquiries.

• Collaborate with physicians, social workers, chaplains, nurses, and other members of the hospice team.

• Make clinical judgments based on patient condition and care plans.

• Provide instructions for managing symptoms such as pain, difficulty breathing, nausea, anxiety, and other issues.

• Offer emotional support and guidance to families and caregivers.

• Educate patients and families about end-of-life care, medication administration, and comfort measures.

• Respond to emergencies, guide caregivers in symptom-control techniques, and dispatch hospice field nurses as necessary.

• Organize after-hours visits by hospice field nurses or other team members.

• Ensure prompt ordering and availability of medications and medical supplies.

• Record triage interactions and nursing assessments in the electronic medical record (EMR).

• Complete workflow tasks.

• Provide comprehensive reports to registered nurse teams, hospice physicians, and case managers.

• Update care plans to reflect changes in patient condition and maintain interdisciplinary communication.

⛳️ Requirements

• Associate’s degree in nursing.

• Current RN licensure with an active multistate (compact) license is required, or a willingness to acquire one.

• Current CPR certification.

• Valid driver’s license and reliable transportation for potential home visits.

• Bachelor’s degree is preferred.

• A minimum of two (2) years of experience in hospice or palliative care is preferred.

• Experience in telephone triage, critical care, or emergency care is preferred.
