Care Navigator

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Act as an advocate for patients and their families throughout the entirety of their healthcare experience.

• Inform patients and caregivers about the services available, eligibility criteria, care objectives, and options for transitioning.

• Recognize instances where patients could benefit from transitioning among rehabilitation, home health, and hospice services.

• Facilitate smooth transitions between service lines to minimize care gaps and prevent unnecessary hospital admissions.

• Cultivate trustworthy relationships with patients, families, healthcare providers, referral sources, and community partners.

• Work collaboratively with nurses, therapists, social workers, case managers, physicians, and clinical leadership.

• Offer empathetic guidance and support during transitions in health status and disease progression.

• Monitor patients who are at risk for decline or hospitalization and relay concerns to the relevant clinical team.

• Promote a positive experience for patients and families throughout the entire continuum of care.

⛳️ Requirements

• Preferred qualifications include being a healthcare professional, such as a Social Worker or Skilled Nurse.

• 2 to 3 years of experience in home health, hospice, rehabilitation, case management, care coordination, or post-acute care is required.

• Excellent communication, relationship-building, and organizational capabilities are essential.

• Must be able to efficiently manage multiple patient needs, calls, and priorities.

• Familiarity with Medicare guidelines and post-acute care transitions is preferred.

• Experience with electronic medical records and documentation systems is required.

• A valid driver's license and dependable transportation are necessary.
