Central Intake & Case Management Coordinator

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Receive, assess, and process referrals.

• Oversee referral activities for designated facilities.

• Maintain databases and tracking systems for referrals.

• Ensure prompt responses to referral opportunities.

• Coordinate all elements of the intake process from referral to admission.

• Evaluate clinical, financial, and insurance details.

• Confirm referral documentation requirements are met.

• Facilitate timely admission decisions.

• Assess referrals for the appropriate level of care.

• Work in partnership with facility clinical teams to determine admission suitability.

• Ensure continuity of care during transitions.

• Assist with transition-of-care initiatives.

• Communicate effectively with families and responsible parties.

• Contribute to initiatives aimed at reducing readmissions.

• Support facilities in meeting census objectives.

• Track admission activities and conversion rates.

• Validate payer source information.

• Coordinate authorization requirements and secure approvals.

• Deliver outstanding customer service.

• Foster positive relationships with hospitals and community partners.

• Collaborate with admissions, clinical staff, business office, and operational teams.

• Encourage teamwork and cooperation.

• Maintain referral tracking reports and admission metrics.

• Monitor referral conversion rates and outcomes.

• Ensure adherence to federal, state, and company regulations.

• Comply with HIPAA standards.

• Safeguard resident confidentiality and privacy.

• Uphold resident rights, dignity, and personal choices.

⛳️ Requirements

• Associate's Degree is required; Bachelor's Degree is preferred.

• Preferred experience in healthcare admissions, case management, discharge planning, or long-term care.

• Familiarity with Medicare, Medicaid, and Managed Care processes.

• Strong skills in organization, communication, and problem-solving.
