This is a fully remote position, open to applicants in Pennsylvania.
• Collaborate with the team to help coordinate care addressing mental health, behavioral, and social needs.
• Facilitate effective communication between patients, families, healthcare professionals, community and state agencies, as well as interdisciplinary team members.
• Assist in the implementation of care plans and the coordination of behavioral health and social services.
• Provide evaluation and treatment support for psychiatric patients, which includes psychological assessments, crisis intervention, case management, patient advocacy, care coordination, and community outreach.
• Contribute to treatment decisions and present treatment plans for approval.
• Obtain and document patient information accurately in the medical record.
• Involve patients, families, and other stakeholders in care delivery and treatment recommendations.
• Collaborate with patients to establish treatment goals.
• Ensure safe and timely patient transfers.
• Serve as an informational resource for division members, patients, and referral sources.
• Assist in maintaining provider schedules.
• Organize appointments and coordinate follow-up treatments.
• Help with insurance verification and authorization processes.
• Support interdisciplinary care throughout the acute episode and into post-discharge.
• Act as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information.
• Identify high-risk patients considering psychosocial and financial factors.
• Assess psychological needs and provide information, support, counseling, care management, and referrals.
• Resolve psychosocial issues related to patient care.
• Develop and execute complex patient transition and discharge plans.
• Bachelor's Degree in Social Sciences (Required).
• Experience: Not specified.
• Skills: Knowledge of Patients Rights, and teamwork capability.
• Must adhere to all organization policies and procedures.
