Case Manager/Social Worker

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct visits with individuals on the caseload and participate in team meetings.

• Attend or complete meetings focused on needs assessments and the development of individual service plans (CSSP).

• Support the planning of new service initiatives.

• Identify residential, vocational, and other essential services tailored to each person's needs.

• Oversee service delivery and ensure the health and safety requirements of each individual are met.

• Complete referrals and provide crisis intervention as necessary.

• Develop waiver budgets.

• Maintain case notes.

• Serve as an advocate for the individuals receiving our services.

⛳️ Requirements

• Must hold a license as a Social Worker (Bachelor of Social Work required for licensure).

• Alternatively, be a licensed Graduate Social Worker (Master’s in Social Work required for licensure) as per the Minnesota Board of Social Work.

• Or possess a 4-year degree in Human Services, Psychology, Sociology, or related disciplines.

• Successfully pass the Department of Human Services background check.

• Successfully pass the Motor Vehicle Background Check and maintain an acceptable driving record in accordance with Company Policy.
