Case Manager/Social Worker

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Identifying resources and services that best align with client needs

• Advocating on behalf of individuals with disabilities and the elderly

• Managing a caseload that includes individuals with intellectual disabilities, traumatic brain injuries, CADI diagnoses, and elderly or alternative care

• Conducting visits to individuals on the caseload and participating in team meetings

• Performing needs assessments and developing individual service plans (CSSP)

• Supporting the planning of new service initiatives

• Finding residential, vocational, and other essential services

• Overseeing service delivery to ensure the health and safety needs of each individual are met

• Completing referrals and providing crisis intervention as necessary

• Creating waiver budgets

• Documenting case notes

⛳️ Requirements

• Must be a licensed Social Worker (Bachelor of Social Work required for licensure)

• Alternatively, must be a licensed Graduate Social Worker (Master’s in Social Work required for licensure) as per the Minnesota Board of Social Work

• Or possess a 4-year degree in Human Services, Psychology, Sociology, or related fields

• Must successfully pass the Department of Human Services background check

• Must successfully pass the Motor Vehicle Background Check and maintain an acceptable driving record according to Company Policy
