
Case Manager – Long-term Care

This is a fully remote position, open to applicants in United States.
• Perform regular visits to members' homes and nursing facilities.
• Evaluate, plan, coordinate, implement, and assess care for eligible members with chronic and complex healthcare, social service, and custodial requirements in nursing facilities or community-based settings.
• Facilitate care coordination across a spectrum of services, addressing members' physical, behavioral, long-term services and supports (LTSS), social, and psychosocial needs.
• Authorize LTSS services following the completion of a comprehensive needs assessment.
• Organize HCBS services, along with Medicaid and DSNP benefits, while evaluating the suitability of care and services in the community.
• Inform members or their caregivers about healthcare needs, accessible benefits, resources, and services.
• Create personalized care plans in collaboration with members or their caregivers.
• Work together with the member's healthcare and service delivery team.
• A Bachelor's degree in Social Work or in health, human, or education services.
• A minimum of 3 years of experience in long-term care, home health, hospice, public health, or assisted living.
• A Master's degree in Social Work or in health, human, or education services with at least 1 year of experience in related fields.
• Current State RN or LPN licensure OR current multistate licensure through the enhanced Nurse Licensure Compact (eNLC) with 2 years of experience in long-term care, home health, hospice, public health, or assisted living OR a high school diploma or equivalent with three years of relevant experience in case management for the aged, including management of behavioral health conditions or individuals with physical or developmental disabilities, or the HIV/AIDS population.
