• Deliver care coordination and case management services for individuals with both acute and chronic health conditions.
• Act as the main point of contact for patients, their families, and diverse clinical teams.
• Provide support, education, and advocacy for patients, ensuring their specific care needs are addressed.
• Organize care plans, track patient progress, and promote adherence to treatment protocols.
• Employ EMR/case-management software to document and oversee patient care.
• Collaborate with healthcare providers and insurance entities as necessary.
• Maintain the organization’s core principles of teamwork, resilience, open-mindedness, ownership, and positivity.
• Education: A Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related discipline is preferred, though not mandatory. Candidates with relevant experience or different degrees are welcome to apply.
• Experience: Background in customer service, care coordination, or case management is preferred but not essential.
• Licensure: Nursing candidates with an active RN license are encouraged to apply, although licensure is not a requirement for all applicants.
• Certifications: Case Management Certification (CCM) is an advantage.
• Familiarity with EMR and/or case management software is advantageous.
• Strong communication, organizational, and time-management abilities.
• Ability to work effectively within a multidisciplinary team.
• A patient-centered approach grounded in empathy, integrity, and innovative problem-solving.
• SALARY - $1,300 PER MONTH
• TYPE OF WORK - FULL TIME (REMOTE)
• WORK SCHEDULE - US TIME ZONE (EAST COAST)
